September 9, 2021 Dear Vendors and Community Partners: In response to the upward trend in COVID cases, the California Department of Public Health (CDPH) has issued a new order on indoor visitations in long-term care settings like our community. Beginning Thursday, September 9th, persons entering a resident’s apartment or working directly with a resident inside the community must either provide proof of vaccination or a negative COVID-19 test result. This includes but is not limited to subcontractors, home health, hospice agencies and essential medical personnel such as PT/OT therapists and podiatrists. Ensuring that all visitors come prepared to meet this new requirement will aid a smooth and seamless entrance. When you arrive for a visit, please check-in at the front desk. • All visitors must provide proof of vaccination or a negative COVID test upon entrance. • Proof of vaccination may be in the form of: o a vaccine card o a photo or screenshot of a vaccine card on a phone o documentation from a healthcare provider o digital record that includes a QR code (myvaccinerecord.cdph.ca.gov) • Proof of a negative COVID test result o must be within 72 hours of the visit o If a visitor has a history of COVID-19 within 90 days of the visit, they may present documentation of release and recovery from COVID from your healthcare provider instead of testing results o our community cannot provide testing to visitors o test results will be documented but expire after 72 hours All visitors must also, wear a mask, properly worn over the nose and mouth. A surgical mask is highly recommended. Maintain physical distance from residents, team members, and other visitors while in the community.
