What\u2019s sometimes overlooked is that people with advanced dementia lose the ability to chew and swallow without choking and aspirating food into their lungs. Years ago, doctors often recommended surgery to put a feeding tube in the patient\u2019s abdomen. After years of studies, though, <strong><a href=\"http:\/\/www.nytimes.com\/2016\/08\/30\/health\/tube-feeding-dementia-patients.html?_r=1\" target=\"_blank\">feeding tubes are no longer recommended<\/a><\/strong> for patients with late-stage dementia, and the New York Times says more families are opting to stick with hand feeding instead. Here\u2019s why.<\/p>\n<p>Non-profit group Choosing Wisely notes that <strong><a href=\"http:\/\/www.choosingwisely.org\/patient-resources\/feeding-tubes-for-people-with-alzheimers\/\" target=\"_blank\">feeding tubes do have their uses<\/a><\/strong>. Patients who can\u2019t swallow (but don\u2019t have a life-ending illness) and people recovering from trauma or surgery can benefit from the nutrients a tube can deliver. For people with advanced dementia, the outcomes are different.<\/p>\n<h2>Feeding tubes carry some risks for late-stage dementia patients<\/h2>\n<p>The American Geriatrics Society\u2019s position is that \u201cwhen eating difficulties arise, feeding tubes are not recommended for older adults with advanced dementia.\u201d AGS says that\u2019s because tube feeding these patients raises their risk of<\/p>\n<ul>\n<li>agitation, which can require \u201cphysical and chemical restraints\u201d to subdue<\/li>\n<li>pressure sores, which can be painful and lead to infection<\/li>\n<li>more medical procedures related to problems with the tube itself, such as trips to the ER to unblock a clogged tube<\/li>\n<\/ul>\n<p>AGS also says \u201cfeeding tubes should never be viewed as a cheaper, more efficient way of feeding,\u201d and that the choice of whether or not to have a feeding tube surgically inserted\u00a0should be up to the family based on advanced directives or patient wishes. Doctors may offer the option, but the final decision should be based on the patient\u2019s wishes.<\/p>\n<h2>Feeding tubes don\u2019t help advanced dementia patients live longer<\/h2>\n<p>Dementia patients who develop trouble swallowing are making what the AGS describes as the \u201ctransition from advanced dementia to the end of life,\u201d and about half of these patients will die within six months. This is a hard, heartbreaking thing to accept, and it\u2019s natural to hope that nutrition delivered by tube would give patients more time. However, studies of healthcare outcomes have found \u201ca short <a href=\"http:\/\/www.jamda.com\/article\/S1525-8610(08)00412-X\/fulltext#sec4.3\" target=\"_blank\"><strong>survival time after feeding-tube insertio<\/strong>n<\/a><strong>.<\/strong>\u201d For patients who needed their feeding tubes replaced or re-positioned, the median survival time was less than two months.\u00a0The Alzheimer\u2019s Association also says\u00a0that<strong> <a href=\"http:\/\/www.alz.org\/documents_custom\/statements\/Feeding_Issues.pdf\" target=\"_blank\">feeding tubes don\u2019t reduce the likelihood of pneumonia<\/a><\/strong>\u00a0caused by food aspiration.<\/p>\n<h2>Alternative feeding approaches for people with severe dementia<\/h2>\n<p>Both the Alzheimer\u2019s Association and the AGS recommend feeding patients by hand if the patient is comfortable with it and can safely take in some food or liquid. In part, this is because meals are\u00a0a sensory pleasure and a time to have social contact with others \u2013 two things tube feeding can\u2019t provide. For patients whose <strong><a href=\"http:\/\/consumerhealthchoices.orghttps:\/\/s3.us-west-2.amazonaws.com\/img.prod.aplaceformom.com\/main\/uploads\/sa-blog\/2013\/05\/ChoosingWiselyFeedingTubeAGS-ER.pdf\" target=\"_blank\">appetite is declining<\/a><\/strong>, Consumer Reports Health suggests that caregivers:<\/p>\n<ul>\n<li>Look for and treat underlying causes of appetite loss, such as infection or depression<\/li>\n<li>Ask the doctor if medications may be making a weak appetite worse<\/li>\n<li>Make a dental appointment to look for potential tooth and denture problems<\/li>\n<\/ul>\n<p>Correcting these problems can help boost a patient\u2019s appetite and improve their overall comfort.
